CC (chief complaint): This is a brief statement identifying why the patient is here in the patient’s own words, for instance, “headache,” not “bad headache for 3 days.”

HPI: This is the symptom analysis section of your note. Thorough documentation in this section is essential for patient care, coding, and billing analysis. Paint a picture of what is wrong with the patient. Use LOCATES Mnemonic to complete your HPI. You need to start every HPI with age, race, and gender (e.g., 34-year-old African American male). You must include the seven attributes of each principal symptom in paragraph form, not a list. If the CC was “headache,” the LOCATES for the HPI might look like the following example:

Location: head

Onset: 3 days ago

Character: pounding, pressure around the eyes and temples

Associated signs and symptoms: nausea, vomiting, photophobia, phonophobia

Timing: after being on the computer all day at work

Exacerbating/relieving factors: light bothers eyes, Naproxen makes it tolerable but not completely better

Severity: 7/10 pain scale

Current Medications: Include dosage, frequency, length of time used, and reason for use. Also include over-the-counter (OTC) or homeopathic products.

Allergies: Include medication, food, and environmental allergies separately. Provide a description of what the allergy is (e.g., angioedema, anaphylaxis). This will help determine a true reaction versus intolerance.

PMHx: Include immunization status (note date of last tetanus for all adults), past major illnesses, and surgeries. Depending on the CC, more info is sometimes needed.

Soc & Substance Hx: Include occupation and major hobbies, family status, tobacco and alcohol use (previous and current use), and any other pertinent data. Always add some health promotion questions here, such as whether they use seat belts all the time or whether they have working smoke detectors in the house, the condition of the living environment, text/cell phone use while driving, and support systems available.

Fam Hx: Illnesses with possible genetic predisposition, contagious illnesses, or chronic illnesses. The reason for death of any deceased first-degree relatives should be included. Include parents, grandparents, siblings, and children. Include grandchildren if pertinent.

Surgical Hx: Prior surgical procedures.

My experience during adolescent idealism was a little shaky. I remember when I was about 13 experiencing many hormonal changes, specifically, very severe acne. I was so defensive about it all the time at school because other students would just stare at my face and I’d become so upset. It just got to the point where I’d get home from school and lock myself in my room. I hated to even come outside and be seen and just wanted for the acne to go away. My parents had not idea how to deal with some of my concerns during that time like the acne problem. They finally tried to help when one day I came home crying because some kids had made fun of me calling me a “pizza face.” I ended up going to a dermatologist and it did help me with the acne. My adolescent criticism was hating being criticized and made fun of because I had so much acne. It often affected me obviously in a begative fashion because my self esteem was basically on the floor and it geared me to not want to socialize much with anyone. My personal fable was that I was this good looking young man with the best personality and everyone wanted to be my friend. I didnt wan’t to continue to believe I had any further acne problems or anything so I would lead myself to believe I had changed to this new guy. So, basically I dismissed what my mind was feeling. Luckily, as the years went on, I grew out of that stage and slowly became an adult. I am a much older person then most in this class and I feel that most people don’t really become mature between the ages of 20-29. I think that that people are more mature than when they were in their teens but quite enough to really be an adult with enough life experiences to say that one is a mature adult. I think one becomes more and more mature with life experiences and aging. I am of the opinion that people don’t really fully mature until the mid thirties. I think when a person experiences life and endures good and bad things, you become better at being mature.

Building a health history: Asking difficult questions

Much of an archeologist’s work is done under the mantra “proceed with caution.” Archeologists must dutifully secure permissions to access sites. They also must exercise extreme caution when excavating or analyzing in a lab to avoid potential damage to historical artifacts.

Likewise, nurse practitioners must proceed with caution when building a patient’s health history. Important questions can be difficult for both nurse and patient. Care must be taken to approach such questions with dignity, tact, and respect to create an environment conducive to productive conversations.

For this Assignment, you will develop a script to be used to interview a volunteer serving in the role of patient.

To prepare:

  • Describe the components of a complete gynecologic health history. Include considerations for special populations such as LGBTQ+ individuals.

  • What health maintenance guidelines should be included for initial and follow up might be needed for follow-up assessments? (i.e., bone density test, Gardasil vaccine, shingles, etc.)?

  • What questions would you consider in your patient’s assessment? For example

    • What is your patient’s living situation?

    • Do they have stairs?

    • Do they live by themselves?

    • Do they have a working refrigerator?

  • Create your own script for building a health history and use the Health History Template for guidance (consider the type of language you would use to help your patient be more comfortable). As you create your script, consider the difficult questions you want to include in your script.

  • In addition to your script for building a health history for this assignment, include a separate section called “Reflection” that includes the following:

    • A brief summary of your experiences in developing and implementing your script during your health history.
